Family Mentoring - key achievements 25/26

  • 351 individuals supported
  • 1,806 mental health interventions delivered
  • 209 referrals received, exceeding the target of 70
  • All contractual KPIs exceeded, including wellbeing, confidence and behaviour-management outcomes
  • Outreach Support Groups provided early help for families on the waiting list
  • STOP and FLASH programmes delivered with strong engagement from 47 families
  • Staff upskilled through specialist training in self-harm, ADHD, domestic abuse, sensory processing and trauma-informed practice

Social Prescribing and hospital discharge

Our Social Prescribers based in the Third Avenue Surgery on Canvey Island supported 575 residents within the year. Our Community Support Advisors based in Southend Hospital supported 520 Castle Point residents with their discharge; of them only 2% returned to hospital within 30 days (the usual readmission rate is around 14% within 30 days).

Information and advice

Our Essex Cares Limited Information and Advice Officers supported 896 Essex residents; 400 of these were supported into new benefits, increasing their income by £114.60 per week (£5,959.20 per year). The yearly total of additional benefits brought into Essex was £2,383,680.

Befriending

Our Befriending service supported 271 people across Castle Point, Rayleigh and Rochford, aged from 26 to 104, who were experiencing loneliness.

Networking

Our three networking events involved 158 organisations and around 300 individuals; one event allowed the public to exchange their slippers to prevent falls, giving out 100 pairs of slippers for free.
